Basis-32 is a 32 item survey used as a Behavior and Symptom Identification Scale. Over the years, it has been the leading behavioral health assessment tool and used in over 800 locations across the U.S., Canada, and all over the world. It is known to be valid and comprehensive assessment. The survey measures the change in self-reported symptoms and the problem difficulties over the course of a person's treatment. The whole survey is from the patient's perspective and is typically given at admission to and discharge from a hospital.
The scoring of the 32 items provides indication of how a patient feels before and after receiving care from doctors. It scores the difficulties that the patient has over a one week period. The scale range that the BASIS-32 survey uses is a five point scale from no difficulty to extreme difficulty. There are five subcategories to the survey: Relation to Self and Others, Depression and Anxiety, Daily Living and Role Functioning, Impulsive and Addictive Behavior, and Psychosis.
The survey is used for anyone 14 years or older that is receiving health treatment of any level or type of program.
