Patient-Centered Medical Home (PC-MH)

Description
Patient-centered medical home (PC-MH) is a new model for health care. Patient-Centered Medical Home is when all of a patient’s healthcare providers cooperate for the good of the patient. A primary care provider may refer a patient to different specialists for certain conditions. Through EMRs and EHRs health care professionals will be able to more efficiently communicate with each other about the same patient in order to ensure the highest quality of care.

Application
The idea of Patient-centered medical home has become much more widely used by primary care doctors. Using multiple physicians as a means to ensure a higher quality of care for each patient. When doctors aren’t giving singular care to each of their patients, they have more time to see more people as well as provide a higher quality of care
